Resident Service Coordinator (RSC) is here to assist residents and their family members with information about supportive access to local services and resources that can assist the residents to achieve their life opportunities objectives.

  • The RSC welcomes new residents and maintains contact with existing residents.
  • Identifies, assess, selects and helps to develop and maintain referral partnership relationships with local service resource agencies that effectively assist residents to achieve their life opportunities objectives.
  • Provide supportive linkages between residents and referral agency staff when residents or agencies request assistance
  • Identify and assess individual and family needs when appropriate; inform the resident of available resources and provide support in accessing services successfully
  • Help to facilitate tenant meetings and community-organizing and social activities for the residents
  • Develop supportive professional relationships with residents that help them enhance the quality of their lives, empower them and encourage them in taking the steps to achieve self-sufficiency

Assisted Living Services provides nursing and personal care to individuals living at Tower One/Tower East. A Plan of Care is developed by an Assisted Living Registered Nurse and a service level is assigned to the resident. Our Assisted Living nursing team understands that each person is unique; as a result, the level of care varies according to the needs of the resident. Each level provides care that is tailored to meet the resident’s needs.

Services include:

  • Medication Management
  • Nurse (s) assess Health Status via monthly wellness visits and as needed
  • Nurse (s) act as liaison between resident’s healthcare providers and caregivers
  • Continuity of care between assisted living services and home care services
  • Wellness Education and Advocacy
  • Registered Nurse is on-call 7 days a week, 24 hours a day

Our Assisted Living Services Program are staffed with compassionate, trained Aides that provide personal care services in order to ensure that residents are able to maintain active, independent lifestyles.

  • Bathing, Personal Care and Dressing
  • Medication Reminders
  • Assistance with Meal Preparation
  • Safety Checks
  • Socialization
  • Escort within Towers Community

Geriatric Adult Psychiatry (GAP) Provides mental health and dementia care to Towers Residents as needed. 

Project ElderCare, The Hospital of Saint Raphael's award-winning Project ElderCare has six clinics that provide primary care and social services to hundreds of seniors in easy-to-access community settings, including elderly housing complexes. Tower One houses one of these clinics.

Project ElderCare offers:

  • Convenient access to health care, specialized for seniors
  • Complete geriatric primary care
  • Opportunities to develop long-term relationships with permanent staff, including geriatric physicians, physician assistants and social workers
  • Social services, including counseling for personal and financial problems
  • Labs drawn at clinic
  • Foot care by podiatrists
  • Less wait for follow-up appointments
  • Coordination of care with your medical specialist
  • Patients can be seen on other days at other clinic sites for urgent problems
  • Health screenings including blood pressure, blood sugar, vision and hearing.

Towers Alert Line is a 24 hour emergency response monitoring system within your own Towers home. With just the push of a button (on a pendant or wrist band one is wearing) provides you safety and comfort in your own apartment.

Heritage Rehabilitation Corporation has collaborated with The Towers to bring on-site rehabilitation services to its community. As a national provider of physical, occupational and speech therapy, Heritage has a formal therapy program that promotes aging in place, while using an innovative program to engage residents at various points in their aging process, meaningfully improving quality of life along the way.
